TYPES
OF IMPLANTS AVAILABLE – PROS AND CONS FOR BREAST
AUGMENTATIONS.
In the United States, silicone gel breast implants
have, once again, been made available to women desiring
cosmetic breast augmentation. Silicone gel implants
have a more natural feel or texture and usually
do not ripple or deflate. The gel portion has gradually
evolved into a thicker memory type of gel that remains
cohesive even in the unlikely event that the implant
is damaged. Silicone gel implants are more expensive,
however.
Saline filled breast implants have the advantage
that the volume of saline can be finely adjusted
during surgery to make the breast (average volume)
as even as possible and they can be inserted through
a slightly smaller incision.
Because the shape of the augmented breast is primarily
dependent upon the skill of the surgeon, the appearance
is almost the same for both types of implants. The
most important difference between these two types
of breast implants is that saline filled implants
have a greater tendency to develop rippling, especially
in women who have very little breast tissue of their
own to cover the implant. Rippling after saline
implants is not a problem for most of Dr. Davidson’s
patients because he places the saline implants behind
the pectoral muscle, inflates them fully, and does
not detach or cut the attachment of the pectoral
muscle to the breast bone.
Saline filled breast implants have the advantage
that the volume of saline can be finely adjusted
during surgery to make the breast (average volume)
as even as possible.
Dr. Davidson, who has an extremely large, 30 year
experience with both silicone gel and saline-filled
breast implants, notes that the patient satisfaction
is extremely high and about the same when the two
groups of patients are compared.
Dr. Davidson prefers round, smooth surfaced breast
implants over breast implants which are tear-drop
in shape and have a textured or roughened surface.
Dr. Davidson’s experience has led him to the
opinion that round smooth implants are softer and
more mobile (similar to natural breasts) and have
a much lower deflation rate. Also, rotation of the
smooth breast implant is not detectable. In contrast,
rotation of a tear-drop shaped or anatomical breast
implant could result in the breast implant being
upside down, with the fuller part at the top. If
the surgery is done well, round breast implants
should look just as natural as teardrop shaped breast
implants.
